Each Master Pooled Trust Agreement has a Joinder Section. The Joinder Section is located at the back of this document and needs to be completed for each individual.
The Master Pooled Trust Agreement is approved for use by the appropriate legal body within each state. For example, though Federal Law sets the backdrop for Special Needs Trusts some differences apply from state to state. For example, different Master Pooled Trust agreements exist between Colorado and Pennsylvania due to separate sets of rules regarding Medicaid Paybacks--as a result, different Master Pooled Trust documents needed to be approved in both States.
The appropriate Master Pooled Trust Agreement can be downloaded and copied for each individual.
The LET Enrollment Tip Sheet Guide is a reference document that provides step-by-step instructions for completing the Joinder Section. When completed and signed it enrolls the individual!
